Student Sign Up Form

Want to register for tutoring as a student? Fill out this registration form with a parent/guardian, and we’ll match you to a tutor for the subjects you need assistance in. Students will be paired with a tutor matched for their specific needs, and will have sessions at least once a week. Any information that you provide on this form is completely confidential within the Connect Me organization and will NOT be released to an outside source.
